Join us as a Client Care Specialist, where you’ll take the lead on care planning, Caregiver management, and ensuring every client feels valued and well-supported.

What You’ll Do

  • Assess Client Needs
    Conduct in-depth evaluations of new clients to determine their specific needs, preferences, and care requirements.
  • Develop Care Plans
    Create, implement, and regularly update personalized care plans that meet regulatory standards and reflect our commitment to exceptional care.
  • Lead & Support Caregivers
    Manage and train a dedicated team of Caregivers—offering mentorship, performance feedback, and ongoing education.
  • Maintain Client Relationships
    Act as the main point of contact for clients and their families. Promptly address concerns, resolve issues, and ensure everyone feels heard and supported.
  • Collaborate with Partners
    Work closely with external healthcare providers and community resources to ensure our clients receive coordinated, comprehensive care.
  • Keep Detailed Records
    Maintain up-to-date, confidential client files and track all relevant documentation in compliance with industry regulations.
  • Provide Leadership Updates
    Regularly communicate client progress and caregiver performance to company leadership, highlighting successes and addressing any roadblocks.
  • Drive Recruitment Efforts
    Assist in identifying and onboarding new Caregivers as our team grows.
  • Stay Current in the Field
    Keep pace with the latest home care trends, best practices, and regulations. Participate in ongoing professional development opportunities.
What We’re Looking For

  • Clinical license preferred (RN, LSW, PT, OT) – We strongly encourage RNs to apply. Candidates must hold a clinical license or have equivalent experience in case management, senior care, or a related healthcare field to be considered for this role.
  • Experience managing CNA's, HHA's, or similar roles.
  • Solid understanding of home care regulations, industry standards, and emerging trends.
  • Outstanding communication and interpersonal skills with a knack for problem-solving.
  • Proven ability to handle multiple priorities in a fast-paced setting.
  • Dedication to delivering top-notch client care and customer service.
  • Proficiency in Microsoft Office and electronic client management systems.
  • Valid driver’s license and reliable transportation.
